Description: When we think of emergencies, flashing lights, sirens, and fear are things that come to mind. For many people, there exists a happy assumption that when trouble strikes, there are people in the world who are well-trained and down-right similar to “superheroes” who will be there to save the day. Essentially, they are mysterious individuals who we forget about until we need them. The reality is that these heroes may be the lady next to you in the grocery store, or the man fixing your sink when it decides to leak. Public safety dive teams are like any other public service group. They are often made up of community members who have decided they want to provide service to others. Sometimes they are paid for their work, but often they volunteer. They give time on weekends, learn interesting skills, and they work hard to be successful. In contrast, they are not always specialists in the subject matter you would expect. In the past year, we have seen rescue scenarios take place on the world stage. The world community sometimes provided praise, but also offered criticism of who was sent to help, how help was provided, and even simple actions that were, or would be, performed. Everyone seemed to be the “leading expert” on all things associated with helping others. The truth is that sometimes helping others requires different backgrounds from different people. When these unique people are put together, their knowledge, history, and abilities can help to solve problems. Sometimes, the person needed most by a public safety dive team may be a physician, and engineer, or a technical diver with no background in true public safety operations. In turn, these specialists may need the experience of others who understand subject matter such as submerged encapsulation so that everyone involved can maintain safe operational activities. These realities have changed the definition of “public safety dive team.” In 2018, public safety diving evolved into a larger spectrum of capability and potential service. As a dive community, we have to foster this growth and make sure that education, safety, and skill development are established and maintained to support better service to our communities and regions. We must continue to IMPROVE what may be defined as a PUBLIC SAFETY DIVE TEAM.

However, sometimes those models break down and we end up with a surprise – an incident, accident or a near-miss. Knowing how these models work is essential if we want to improve our diving safety. This presentation will challenge how you think you make decisions and what you can do to improve them while you are preparing your equipment to go diving, while you are diving and how to review dives so that lessons are truly learned and not just identified.

Fatalities, fortunately, don’t happen very often, but due to the fear of litigation and the lack of a consistently robust investigative processes, context-rich data and contributory factors often aren’t made available. And yet the same accidents are happening. This presentation will address a number of case studies and show how the industry and the community can make a difference in learning by embracing reporting and just cultures so the context is available. Without context, we can’t determine the ‘how it made sense to the diver at the time’ and if you can’t do that, you won’t be able to learn from the repeated failures which are happening
